Do you know a deserving family who desperately needs a home makeover? Perhaps it's your family? If you would like to nominate your own family or another, then print out our application and send it in, along with a video and/or pictures! The Extreme Makeover team could be coming to your neighborhood!
How to Download the Application:

Mac Users
Control-click on the application link above and choose "Download Link to Disk" or "Save Linked File As..."
You will need Adobe Reader, version 5.0 or higher, to print the application. Don't have Adobe Reader? Get it here!
I'm unable to download this application etc. from work, so I don't know how involved it is. I'm just wondering if we can obtain alot of information to put on an application......... Then everyone send in an application. With all of us here, we could overload the show with our nomination!!!

I read parts of it and printed it up....We need a video of the house, and LOADS of info on her situation, kids, ages, etc living in the house..and it says they receive over 1,000 aps a week, so it could take months before you hear somethng...SO if we are going to do this, we need info...<wink> :D

Yeah, but do they have months to wait? Are they going to have to sell the house when they move her to a different place? Sorry, I'm not up on the situation as to whether it's Jazz's house or if she rents, etc

I saw the application also. We would definately need Jazz's daughter to film the house, and fill out lots of info. But from what I read here about Jazz's situation, this is exactly what they are looking for. I think she stands as good a chance as anyone. Do you think her daughter will help?

I have been lurking in the background through all the discussion, but feel there is a real need to help Jazzie. It seems the show needs something that tugs at the heartstring of America to secure a choosen spot on the show. I think Jazzie would qualify with all the difficulties she faced recently. Her daughter would be the most obvious person to help with the application. I really think we could secure a place for her on the selection process by overwhelming the staff with our power. I have been watching the show and it is usually limited to neighborhood support. This would be different angle for them---worldwide support! Let's give it a try.

Shoot: I wish I wasn't going away so soon. I should be back around the 8th of March. I will do what ever you guys want to help Jazz. Onie this sounds like a wonderful idea and I know that we can do it. Please include me even if I am not here. What a wonderful thing if we could get this done for her. PK.... Maybe you could contact Jazz's daughter and tell her what we need. I think it would be good to send in several + applications. But it would be awesome if we mention on the application that this nomination is sent from all over the world. Somehow include every one from this forum, your contact information to submit with the application. Maybe we could compile a list of names with a paragraph from everyone attach to application.

This is awesome! Jazz owns the house, and she can't sell it unless she gets cash for it because she would have to put too many repairs into it before a lending company would loan anyone the money to buy it and she can't afford that. So she is going to deed it to her grandsons to fix up as they can. So if we could do this that would be just awesome!!

This all sounds like a wonderful idea (as long as I don't have to be in a video, I am way tooooo phobic about that). But I will be glad to fill out a form too. I do suggest we make sure her daughter is willing to go along with this before we get too involved in the project though.

I agree with you Sue...We definetly need her daughters help with his..and I think instead of sending in 100's of app's for the same thing, we get one person to write a letter to go along with it, and include everyones name and where we live...to support this claim...Cause if we all send one in, they might not all get looked at the same time, and that would defeat this purpose...Does anyone know if there is a number we could call for dire emergencies...? Might go faster..<wink>
Just my thought on the subject...This is too perfect of a solution to waste... :D

I hate to be a party pooper but let's step back and look at this from a realistic stand point. Who pays the taxes on the income? I know you pay a huge tax on winnings (i.e. lottery winnings) so will this be considered income? Then her insurance will now go up because they don't build just a nice little cottage, they go BIG and BEAUTIFUL! Then every year from now on, her own personal property tax will go up big time. It's a nice sentiment and a truly wonderful idea if all goes well, but will she be able to maintain it or are we creating a bigger burden for her. In the end, will she then have to sell her home or risk losing it?

was wondering the same thing. I like this idea but I would hate to put her in another bad situation. We need to find out more first. ;)
Gee a person just cannot get ahead. Heres what we do in Canada. We sneak and fix inside up and leave the outside to look shabby. Taxes stay the same.:).When I was tearing down my home which took 2 years I still had to pay full taxes even though it was gutted and no windows.As soon as it was down then they changed and lowered the taxes.I got lucky as someone knocked on my door and ask if they tear it down can they have all the lumber and I will clean as I go.Now you are not going to believe this one but then we had a big hole where the basement was.Well I got lucky as the neigbour got his driveway paved.His driveway is directly across from ours and we ask for the soil removed.yep we got 8 tamtum truckloads full. Saved about $800.Filled our hole.
I do hope this with Jazz can be fixed.It is a shame as she is a good soul and one of the best Angels we could ever meet on this earth.We love her dearly.

A lot of times what happens is the builders pay off any mortgage on the house-and donations come in to help with the taxes and stuff. But-to be safe it sure would be a good idea to find out how it all works. Plus like you said they don't build em small-and they would have all top of the line stuff and any kind of aparatus needed for Andrew!! And her other 2 grandsons would be able to stay in the house with her too!! I wish we could get this off the ground!

They pay off the mortgage, but there is still taxes. Donations only go so far. Who pays the taxes next year, and the insurance? We really need to be SURE before we proceed. Is there any kind of trust fund to take care of Andrew? Any government departments paying for HIS home and board that could be used to pay the taxes and insurance. The TV people don't care about next year, they care about ratings. I keep thinking of that big HGTV DREAM HOME giveaway. I was looking at it. NOT ONE winner has EVER been able to keep the home. They all have to sell. So is it feasible that Jazz could sell this Makeover home if needed and purchase something she could truly afford? Would she need to? I just think we should be sure of all the future problems that could occur.

I am interested in this but am concerned as 'hope' is that we would just create a bigger burden for Jazz.(that's why I was wondering if there was a smaller community group of volunteers that might like to make this a project.....don't know how things work over there). Sorry being so far away there is not much I can do.

I really want this to work. I just want someone to be watching out for Jazz in this. Maybe the lawyers at Extreme Makeover have a form (with small print of course) that explains the ramifications of doing this.

I just wanted to let you all know that you are complete angels. I Am not in Alabama right now. My husband and I had to return to Oregon on Monday. We are moving to Alabama May/June after our kids finish the school year. Mother is going into a rehab facility for about 1 month so that she can improve her strength and learn so techniques for using a walker and exercising. My aunt is in Alabama with her still...you can reach her at 256-492-1860. Her name is Cathy Battles, if you want to do the application for the home makeover you can call her for the video footage...i think she could help.....
my grandmother owns the house and all they pay are property taxes each year.

YES they DO pay off the House..UNLESS it's already paid for in which case they pay the taxes ( I Believe only the land taxes, not school but don't quote me on that) for X amount of years...now they had one show a while back where they paid the taxes for the life of the man they built the house for ON TOP of paying off his home.
You have to remember guys that Jazz isn't a young mom, so the house they build her isn't going to be the ritz, granted it will be bigger than what she has now but it won't be the grand dame we are use to seeing them build for big families.
I honestly believe given Jazz's situation with HER health and the fact that she has Andrew with HIS health we have an excellent chance of pulling this off...I mean come on...If we didn't know her and her story was on TV..wouldn't it make you cry your eyes out for the hour the show is on? I say we go for it...we can always ask the producers of the show the legal stuff about the taxes and whatever else we can think of along with her family. I cry every night when I pray for Jazz...her story (life) is heart wrenching, makes me thank God for my problems cuz they could be a whole lot worse! This is only my opinion.

I guess at this point we should start the application process and I think that I will call her sister and see if she is able to videotape the house. I am not sure how long she will get to stay with Jazz so I think I will call her today and see what she thinks of the whole idea. How does that sound?? good news to hear about them paying the taxes for a while. I was just getting caught up on this thread for the past couple of days and thought I would throw this in to ease concerns. Someone mentioned that right now she can not even sell the house (which she can't live in either) unless it was for cash...if it was fixed up, at least she would be in a position to sell it if needed. financially in the long run, having the makeover can only help. That is a step back and removing the emotional aspect of it being her home but from a fiscal approach it puts her on a more sound footing regardless.

Alright girls I talked to Jazz's sister Cathy a few minutes ago and she is very willing to fill out the application and to take pictures/video of the house and family. So next we have to all coordinate writing our little paragraphs of who we are, where we are from, how we know Jazz (Linda), and why we are nominating her for a makeover!! Maybe if you could all write your own paragraph and email them to me-I can print them all off and mail them to Cathy to include with the application. Please let me know what you think about this idea.

Yes I do think that you should each include your contact info. I do believe that if they do pick her for the show they will want to contact at least some of us that sent in why we want her to get a new home.

If everyone could do it as soon as possible that would be helpful. I already mailed the application off to Cathy because Jazz's printer wasn't hooked up and she had no way to print it off. So when she gets it she will start filling it out and than she will have to take the video. So as soon as everyone can-please start writing your stories and send them to me by email. I could give a rundown of what I know-but first I think I will surf some old posts. I remember one she posted with a lot of details in it I think. And I think that if you all do it in a word program that would be fine. I have word and than I can print them all out. I will get you some background info soon. Thank you everyone.

Girls-sorry I have been busy. I will surf some posts tonight and get you some info. But to start with you can write about how you met her on here and how she is always so caring of everyone. Andrew was about 2 years old when he had his near drowning accident, and Jazz has taken care of him ever since. Through the years Lori has helped, and they had nurses come in to help so the family could have some breaks during the day. But I will look for more for you. Thank you all for helping with this by writing your letters. I really hope that she gets picked.:)

I've been searching the threads ( sorry I don't know how to link to any one in particular) but I searched all threads for posts on "Andrew" and found a really good one from Jazz called "Prayers needed for my grandson". Please read it. It tells alot about her situation and has links to layouts of andrew.

Oh Melanie, I'm so sorry...... I gave PK credit for finding this thread. Either way, the information is excellent!!!! I brought that thread back to the top so that everyone could read through it. It's heart breaking! I actually forgot some of those details until I scanned back through that thread! And the links to the 2 pages that Jazz did on Andrew....... those pages really should accompany the application!
We can have one application, in a big folder, full of letters. We'll this project is a GROUP project that stretches around the world! I'm sure our involvment will be interesting to this project too!
